Insurance
As a service to our patients, we will file a
claim with the insurance company on your behalf for the
services provided to you. Please be aware, however that
ensuring payment from your insurance company is ultimately
your responsibility. The patient is responsible for any
deductible, co-insurance or non-covered items identified by
the insurance company.
Once you have been notified of the portion of your bill for
which you are responsible, we will expect payment in full.
In the event that you are unable to pay the entire balance
due, you will need to contact us to set up a payment plan to
resolve the balance of your account.
If you are not covered under a health insurance plan, the
full amount of your charges will be due at the time of your
visit. If a payment plan is required, please contact at
