Office Policies
You must call at least 24 hours prior to
appointment to cancel or failure to show for an appointment
will result in a $25 charge.
There will be a $25 fee for all returned
checks. After the first returned check, a patient will no
longer be permitted to use personal checks. Payment must be
made by cash or credit card. We accept the following credit
cards:

We ask that each patient arrive at least 15
minutes prior to the scheduled appointment to complete the
necessary paperwork. If a procedure is scheduled, we ask
that someone accompany the patient to drive them home as a
safety precaution. Patients will need to bring a current
list of prescriptions or the prescription bottles to each
appointment.
In December 2004, the DEA posted new rules to
it's policy on writing narcotic prescriptions. For more
information on the DEA rule changes, please visit the
DEA web site. Effective January 1, 2005, we will no
longer be able to dispense 3 months worth of medication at
a time. Patients will be required to make an appointment to
pick up prescriptions each month. These visits will be
referred to as nurse visit and should last no longer than 15
minutes. Office policy prohibits mailing of narcotic
prescriptions. Every 3 months each patient will be scheduled
with a physician or physician assistant to maintain
treatment program requirements.
